Parties

She understands and agrees, by signing the clause, that she cannot change her mind or try to withdraw the contract before the Board has had an opportunity to evaluate and act on it.
Order
It is hereby ordered that Cheyanne Mallas’ Physician Assistant be submitted and accepted by the Board. Cheyanne Mallas’ surrender of her license constitutes disciplinary action against her. It is incorporated into the specification and forms part of Cheyanne Mallas’ license history with the Board.
Cheyanne Mallas lost all of her rights and privileges as a Physician Assistant in California when the Board’s Decision and Order became effective. Cheyanne Mallas must deliver her pocket license and wall certificate, if any, to the Board by April 3, 2023.
If Cheyanne Mallas is a PA When Cheyanneever makes an application or petition for reinstatement in the state of California, the Board will consider it a petition for reinstatement. Cheyanne Mallas, PA Cheyanne may apply for reinstatement no later than two years after surrendering her license.
If a petition for reinstatement is filed, Cheyanne Mallas must follow all applicable rules, regulations, and procedures for reinstating a revoked or surrendered license at the time of the petition. When the Board decides whether to grant or refuse the petition, Cheyanne will be deemed to have acknowledged all of the charges and claims in the First Amended Accusation.
PA Cheyanne must pay the agency $50,191.00 for investigation and enforcement expenses before she can be issued a new or reactivated license.
If PA Cheyanne applies for or reapplies for a new license or certification, or petitions for license reinstatement with any other California healthcare licensing agency, all of the charges and allegations contained in the First Amended Accusation will be considered true, correct, and admitted by Cheyanne Mallas in any Statement of Issues or other proceeding seeking to deny or restrict licensure.
